What if S + V ? という質問文は、
If S + V, (主文の S + V) という文について、主文全体を what で質問する文です。

If I take it and end up missing out on my dream job, (I will regret my decision for the rest of my life).
仮定として、上のような主文をつけてみました。
もし私がそれを選ぶことでずっと憧れていた仕事に就く機会を逃してしまうことになるとしたら、（私は自分の選択を一生後悔するだろう）。

この（私は自分の選択を一生後悔するだろう）という主文の部分について、どうなることだろうか？というふうに質問文で言いたい時に、この主文の全体を what にして、節の前にそのままもってきます。

こうすると、
もし私がそれを選ぶことでずっと憧れていた仕事に就く機会を逃してしまうことになるとしたら、どうなることであろうか？
という質問文ができます。

形としては質問文になっていますが、これについて答えを期待したり想定したりする、というだけでなく、どうなってしまうかわからない、という時にも、質問文の形で what if...? と言うことができます。
