１つ目の文
The number of students passed the test was surprisingly low.
には、
The number of students was surprisingly low.（主文）
Students passed the test.（節）
という２つの文があって、students を示す関係代名詞が省略されていますが、この節を主文につなぐ場合、例のように関係代名詞を省略すると節の部分がわかりにくくなるので、一般的には関係代名詞を省略しないことが多いと思います。
The number of students who passed the test was surprisingly low.
とすれば、節とそれがかかっていることば「students who passed the test」が明確になるので、わかりやすい文になります。

２つ目の文
Among diabetes, cancer and heart disease, heart disease is the ailment that
people are most likely to die from.
には、
Among diabetes, cancer and heart disease, heart disease is the ailment.（主文）
People are most likely to die from the ailment.（節）
という２つの文があって、ailment を示す関係代名詞 that が置かれて節を主文につないでいます。
